Registration page for patients

BPCITracker will monitor healthcare patients following discharge from a hospital or other care setting for 30 or more days (90). Once a user is entered, they (or a designated party) will receive text or voice surveys to check on health status multiple times for 90 days.

All responses will be collected and reported to designated parties (users). Multiple parties (shared responsibility) could monitor and accept response reports for the same patients. System should designate unique patient user identification numbers for each patient, based on name (first/last) and last four didgets of social security number. A patient’s unique identification number should be used for all reporting and communications to secure patient privacy.
