Through an innovative approach, BPCI Tracker was created to improve patient care communications. Our high encounter platform joins advanced SMS (text) technology with a patient’s (or responsible party) existing mobile device. Without equipment costs or extensive training, our system is an immediate solution to patient tracking needs. Patient alert notifications are sent daily (7 days a week @ 9:00 am) via text or recorded voice communication with simple prompts to establish a patient’s status. All patient responses are collected for user reporting. Responses that signal a need for urgent care intervention are immediately notified to users and/or designated parties. In addition to outbound communication alerts, patients can send inbound requests for intervention or to cancel services. BPCI Tracker notifies healthcare providers of patient high-risk events to reduce declines in patient health or rehospitalizations.

Once a new user account is established, you or an assigned party can add and track existing patients with ease. Users will input the following information:
